Comment:

 The new sparsetools does not require sorted indices, so the problem
 appears to have been fixed.

 In [6]: (a + a.T).todense ()
 Out[6]:
 matrix([[ 2.,  0.,  0., -1.],
         [ 0.,  2.,  0.,  0.],
         [ 0.,  0.,  2.,  0.],
         [-1.,  0.,  0.,  2.]])
